Finding the model number
It's worth a look at the rating plate first, because names get reused across generations. The model number is on a rating label on the back or underside of the main unit, also near the barcode on the bottom of the box and on the front cover of the instruction booklet. On Lift-Away uprights look on the pod/body.

When to change them
Shark advises rinsing the foam and felt pre-motor filters monthly and the washable post-motor filter once a year, in water only, and letting them air-dry fully before refitting; tap loose dirt off between washes. Check the instruction booklet as intervals vary by model. (source)
Washable filters go back in bone dry or not at all. A damp one smells, and on some machines it can let the motor pull moisture through.
